• Crafts or spa-themed activities
• Interaction with other children attending the event

I understand that these activities involve the use of cosmetic products and spa tools appropriate for children. AG Little Divas Party Inc. follows strict hygiene practices, and all spa tools used during services are disposable and used only once per child.

While AG Little Divas Party Inc. takes reasonable precautions to maintain a safe and clean environment, I acknowledge that minor reactions or sensitivities to cosmetic products may occur.

FOOD ALLERGIES

Spa parties include food or beverages such as pizza, juice boxes, or sparkling apple juice. AG Little Divas Party Inc. does not prepare food on-site and cannot guarantee that food items are free from allergens. I agree to inform AG Little Divas Party Inc. in advance of any food allergies or dietary restrictions my child may have.

I agree that AG Little Divas Party Inc., its owners, staff, and representatives will not be held liable for minor skin irritations, allergic reactions, accidents, or injuries that may occur during participation while interacting with other children.

I also acknowledge that children attending the party remain the responsibility of their parent or guardian while on the premises.

PHOTO & VIDEO CONSENT
